CONNPACE –
CONNECTICUT PHARMACEUTICAL ASSISTANCE CONTRACT TO THE ELDERLY AND DISABLED
ConnPACE is Connecticut’s state pharmaceutical assistance program (SPAP). It is administered by the Connecticut State Department of Social Services (DSS), http://www.connpace.com/.
ConnPACE subsidizes most prescription drugs, insulin, and syringe costs for low income elderly or disabled Connecticut residents.
ConnPACE "wraps around" the Medicare D Prescription Drug benefit. All people enrolled in ConnPACE who are eligible for Medicare are required to enroll in a Medicare D Prescription Drug Plan.
ConnPACE will pay for Medicare D premiums and deductibles, and will cover prescription drugs while in the Part D "donut hole" ONLY IF THE BENEFICIARY IS ENROLLED IN A "benchmark" Medicare D plan. (For information about which D plans are considered "benchmark" call Medicare (1-800-MEDICARE) or the Area Agency on Aging's CHOICES program (1-800-994-9422).
ELIGIBILITY
- Ages 65+ or, if disabled, ages 18-65 and receiving disability benefits under the Social Security Disability Program (SSD) or the Supplemental Security Income Program (SSI)
- For 2011: 2010 adjusted gross income must be at or under $25,100 for single persons, or at or under $33,800 for married couples
- State resident for at least six months
- Permanent residents (green card) with any length of stay in the U.S.
Note: There is no asset limit. Applicants cannot be enrolled in Medicaid (Title XIX) or an insurance plan that includes prescriptions. People who have health insurance are eligible for ConnPACE when their prescription benefit has been exhausted for the year.
COST
$45 annual registration fee and no more than $16.25 co-pay for each 30 day prescription. (ConnPACE members who have the Part D Low Income Subsidy pay less.)

November 15-December 31.
